With a heavy heart é uma expressão que significa “com tristeza” ou “com pesar”. Exemplos:

“With a heavy heart we announce that Peter has passed away this afternoon.” (É com pesar que comunicamos que Peter faleceu essa tarde.)

“With a heavy heart she said goodbye and left.” (Com muita tristeza ela disse adeus e partiu.)
